CityStreetRN organizes its work through four community-outreach programs: ShelterCare, Community Events, StreetReach, and Community Partnership Programs.
ShelterCare brings recurring nurse-led outreach into shelters and other trusted locations serving people experiencing homelessness or housing instability. Volunteer nurses provide blood pressure and blood glucose monitoring, health education, and connections to healthcare resources.
Returning to familiar locations helps us build relationships, support health understanding, and create continuity between brief encounters.
Through Community Events, CityStreetRN meets residents at health fairs, cultural celebrations, libraries, churches, neighborhood gatherings, and other public events across Lorain County. Our team offers approachable health education, blood pressure and blood glucose monitoring when appropriate, and information about local healthcare resources.
These events make it easier for residents to ask questions and learn about their health in a familiar, welcoming setting.
Pregúntale a la Enfermera is CityStreetRN's bilingual outreach initiative. It creates a welcoming space where community members can ask a nurse health-related questions and receive understandable guidance in English or Spanish.
StreetReach allows CityStreetRN to take nurse-led education and support beyond scheduled events and into the places where underserved community members are located. Volunteer nurses and support-team members offer compassionate conversation, practical health information, mini first-aid kits when available, and connections to appropriate healthcare resources.
StreetReach is built on consistency, dignity, and the belief that support should not depend on a person's ability to enter a traditional healthcare setting.
Community Partnership Programs are created with shelters, healthcare organizations, schools, libraries, faith communities, businesses, and other local groups. Each partnership responds to an identified community need.
Working together helps CityStreetRN extend its reach while keeping services relevant and connected to existing local resources.
