What We Do: Services for the Homeless

Our street-based nursing model focuses on prevention, education, and access to care. Services we currently offer include:

Vital Signs Checks (blood pressure, glucose, pulse ox, temperature)
Medication Education (how to take meds correctly and safely)
Distribution of Medication Lock Bags (free, tamper-resistant storage)
Diabetes & Hypertension Education
Health Insurance Navigation (Medicaid, CareSource support)
Free BP Monitors (for eligible clients)
Doctor Appointment Scheduling
Follow-Up & Care Coordination
Chronic Disease Prevention Education
We also distribute basic health supplies and educational toolkits.
